Transitional care Support
After an ER visit, hospitalization, or rehab stay, Health Bridge helps coordinate follow-up needs such as provider appointments, medication changes, equipment, home health communication, and care plan updates.
Safety Planning
We help identify common risks in the home, including falls, dehydration, skin breakdown, equipment concerns, and changes in daily function. The goal is to reduce avoidable setbacks and support safer routines.
Provider Visit & Telehealth Support
When care decisions feel complex, we can help families prepare questions, organize concerns, and participate in phone or telehealth care conferences when appropriate. This helps everyone stay aligned with the next steps based on client and family goals of care.
Ancillary Service Coordination
Health and quality of life depend on more than medical appointments. We help coordinate practical supports such as transportation, foot care, dental, vision, hearing, grooming, and other wellness-related services.
Nurse Delegation Support
When nurse delegation is appropriate, Health Bridge supports safe task setup, caregiver coaching, documentation, and ongoing oversight so delegated care remains consistent and appropriate in the home setting.
Monthly Wellness Visits In-Person / Telehealth
Routine nurse-supported check-ins help track changes early, review wellness concerns, monitor trends, and keep families informed through clear updates and monthly wellness summaries.
Care Coordination
